When you need primary care services such as medical visits, follow-up care, annual physicals including well-woman exams, immunizations and ongoing care for chronic conditions, come to GuideWell Primary Care. Exclusively for those with a Blue Cross and Blue Shield plan, age 2 and up. Primary care services are delivered by Baptist Medical Group, an independent care team of doctors, nurse practitioners and medical support staff.

If you need a specialist or a referral, the care coordinator will help you find the right one, discuss your treatment options and concerns, handle the details, plus help you understand the results and next steps.

On-going Health Conditions
Support services for patients with diabetes, high blood pressure, asthma, coronary heart disease, chronic obstructive lung disease and other conditions are available. You'll get one-on-one counseling, dietary help, tips on getting accurate readings, and much more. This is all part of our commitment to help members with ongoing health conditions live better, stronger and more active lives.
